Information Collected

When you submit a Root Session request, the following information is collected: first and last name; email address; current state of residence; time zone and the general session times that work best for you; the session or program option you select; optional information you choose to share about your wellness interests or what brought you to this site; how you heard about Support The Root; and your acknowledgment of the Terms and Conditions, Privacy Policy, and Disclaimer, together with the version acknowledged and the date and time of submission.

The optional narrative field asks that you keep it brief and not include medical records, laboratory results, medication details, or other sensitive health information there. Personal health information is better shared through the secure client portal after your session is confirmed.

When you submit the contact form, your name, email address, the topic of your question, and your message are collected.

Health and Wellness Information

When you complete Your Health Story or participate in coaching, you may voluntarily provide health and wellness information. Support The Root uses that information to prepare for and provide coaching, assess whether the requested support remains appropriate within coaching scope, administer the coaching relationship, and meet applicable business and legal needs. Coaching information is maintained through the Healthie client platform. Support The Root does not sell personal or health information or share it with third parties for marketing. Support The Root is not a HIPAA-covered entity and does not represent that you receive rights under HIPAA through the coaching relationship. Complete confidentiality terms are stated in the agreement governing the applicable service.

How Information Is Used

Your information is never sold, rented, or shared with third parties for marketing purposes.

Tended and Email Communications

If you subscribe to the Tended newsletter, your email address is stored through MailerLite and used only to send the newsletter you requested. You may unsubscribe at any time using the link included in every email. Subscribing is optional and separate from a Root Session request.

Third-party services connected to a Squarespace site may also use cookies or similar technologies under their own policies. Support The Root does not sell personal information or use it for third-party behavioral advertising. SMS Appointment Reminders

If you provide a mobile number and enable SMS appointment reminders through Healthie, your mobile number may be processed in connection with sending those administrative reminders. SMS appointment reminders are separate from marketing communications.

Data Retention

Support The Root retains information it controls only for as long as reasonably necessary for the purposes for which it was collected and for applicable legal, tax, insurance, security, and dispute-resolution needs. Information may then be archived, restricted, or deleted under the practice's retention procedures. Third-party platforms retain and delete information under their own policies and legal requirements, which Support The Root does not control.

Security

Support The Root takes reasonable administrative and technical steps to protect personal information, including using third-party service providers that operate under their own security practices and contractual obligations. No method of transmission over the internet or method of electronic storage is completely secure. If Support The Root becomes aware of a security incident affecting your personal information, it will provide notice as required by applicable law.

Privacy Requests and Applicable Rights

Depending on applicable law, you may have rights relating to personal information Support The Root maintains about you, including access, correction, and deletion. Requests may be submitted through the contact method stated below and may be subject to reasonable identity verification and applicable legal limitations.

Children

This website and all coaching services are intended for adults 18 years of age and older. No information is knowingly collected from anyone under the age of 18.

No Sale of Information

Support The Root does not sell or rent personal or health information and does not share it with third parties for marketing purposes.
