When you submit the booking form on this website, the following information is collected:

  • First and last name
  • Email address
  • Time zone and the general session times that work best for you
  • The session or program option you select on the form
  • Optional information you choose to share about your wellness interests or what brought you to this site

When you submit the contact form, your name, email address, the topic of your question, and your message are collected. Information submitted through the contact form may be used to respond to your inquiry, maintain appropriate business records, protect the security and integrity of the website and practice, and address applicable legal, insurance, or dispute-resolution needs.

2. Health and Wellness Information

When you complete Your Health Story or participate in coaching, you may provide health and wellness information. This information is collected and stored within Practice Better under its security practices and is never sold or shared for marketing. Support The Root may use that information to assess whether coaching remains appropriate, prepare for and provide services, administer the coaching relationship, and meet applicable legal, insurance, security, and dispute-resolution needs. Support The Root is not a HIPAA-covered entity and does not represent that you receive rights under HIPAA through the coaching relationship. Complete confidentiality terms are stated in the agreement governing the applicable service.

3. How Your Information Is Used

Information submitted through the booking form is used solely to:

  • Review and respond to your request for the selected service
  • Determine whether the requested service appears appropriate within the scope of Support The Root
  • Schedule and confirm services
  • Communicate with you about your request or coaching engagement
  • Understand referral sources and website effectiveness
  • Send marketing communications only when you separately choose to receive them

Your information is never sold, rented, or shared with third parties for marketing purposes. It is not used to send unsolicited communications.

4. Newsletter and Email Communications

If you subscribe to the Tended newsletter, your email address is stored through MailerLite and used only to send the newsletter you requested. You may unsubscribe at any time using the link included in every email. Subscribing is optional and separate from the booking form.

  • Formspree: booking form and contact form submission processing
  • MailerLite: newsletter delivery
  • Practice Better: a third-party practice-management platform operating under its own terms, privacy policy, and security practices, used for sessions, scheduling, document delivery, messaging, session access, and payment processing. Payments are processed through Stripe as Practice Better’s payment processor.
  • Google Fonts: web font delivery (typography rendering)

For this reason, services are offered to individuals located in the United States only.

This website and the services of Support The Root are directed to adults in the United States. Support The Root does not target, market, or offer coaching services to individuals located outside the United States.
